NPE in `schema-cartographer.explorer/reduce-enumeration-references-namespaces`

In references-enumeration-namespaces-cardinality-many the code appears to assume that all attr-query-results have a :db/ident attribute. That is not necessarily the case and it results in an NPE later in reduce-enumeration-references-namespaces when calling namespace against nil.

Documentation attribute from schema is missing

This is a useful project as-is but it would be even better if the :db/doc attribute on schema entities were conveyed to the UI. Something makes me think the intent was/is there, but it's not working for me -all I see in the way of documentation is "Automatically generated by Explorer".
