Actually it's the same base code than on sourceforge, I added support for vector (svg and eps) export and colors.

I haven't experienced the issues you mention.

Concerning your first point, make sure php GD is installed.

Concerning your second point, you can just do something like

$code_img = qrcode::png()

end then you can do whatever you want :

header("Content-Type: image/png");
header('Content-Disposition: attachment; filename="qrcode.png"');
echo $code_img;

from phpqrcode.
