You defined server url to be http://tus.test/ but seems your server is not accessible at that url. You can check it by hitting that url in your browser or using curl in command line.

@ankitpokhrel webserver is accessible. If I remove it all and add a simple index.php it works. Maybe some issue with the .htaccess rules?

Never mind, it was something related to uploading the same thing, with the same key and name multiple times. Thank you all!
