This might be a use case for a an optional block index located before the blocks. If a streaming block is added to the file then no further blocks can be written anyways, so it might be an argument for inserting the block index at the beginning. Granted, in the off chance several large blocks are written before the streaming block is added this can still be an expensive operation, but that might be a corner case. I don't know if it's worth the added complexity.
