Looks like a timing issue - I can reproduce it with the leaderboard example by refreshing a couple times. The collection elements are getting returned by the initial fetch, but then they're also triggering added callbacks.

I might be missing something, but I think I can just take out lines 45-47. The initial fetch/insert isn't necessary since added callbacks get triggered for the initial data. I want to do some more testing before changing this though.

I think you're correct, and I see a similar approach used by the counts-by-room official Meteor example.
