Current state of the document outline, more things to work on:
- async refresh of the document outline pane.
- collapsed nodes should be parsed on demand (when the user is expanding that node), they will be auto-expanded when the user is filtering.
- reuse view-models so that we don't need to create them on each key-stroke.
