BEP 52 is designed to be a fairly narrow change mostly affecting the file representation in the info dictionary and incorporating a subset of the fast extension. So it is compatible with most previous BEPs that do not touch these parts. That includes BEP 10.

What would be the benefit of that if a client could just announce an empty set of supported extension messages? Or a completely empty handshake dictionary for that matter. None of the keys are mandatory. Minimal support is almost the same as no support.

With BEP 10, the set of extensions varies dynamically: there's the initial
state just after the BitTorrent extension, when no extensions have been
advertised yet, then the state after the first Extended(0) message. (In
principle, a client could further change its set of extensions later, but
of course that would be racy and I don't know of any client that does
that.)
What I'm suggesting is that there should be just one handshake at the
beginning of the connection. This way, the set of extensions is constant
for a given connection, and the state machine is simpler.

That would require changing the wire protocol more significantly since it would obsolete the bitfield and one of the message type bytes. BEP52 is designed to be backwards-compatible (hybrid torrents, same wire format) and minimize additional implementation complexity for existing clients, which means keeping those things anyway.
I agree that it would make sense to integrate it in a greenfield design.
