It seems that the database structure was not updated, have you cleaned your cache ?
If so, can you check the value of version in the core_resource table for customgrid_setup ?
And if it's already 0.8.9, the setup was certainly interrupted because of a race condition, you can try to force the new version to be reinstalled by setting 0.8.4 in version and data_version (the safest way to do it is when your site is on maintenance and only your IP can access it, to avoid those possible race conditions).
