Looks fine visually. A few UX-related questions:
- How does someone arrive on this page? Does it require sign-in?
- Can one review the CLA doc anonymously?
- Can one return to this page or a read-only version of it after submitting a CLA? If so, what might they see? (Status? Date completed?)
- Is there anything else one can do on this site (initially of course) as a registered/signed-in user?
- What happens if a link gets clicked (e.g, one of the ones at left) while the form's being completed? Do we just pop a new tab to some wiki content?
- Will we validate the submitted Jira ID? (I'm not even sure we do so today; just asking.)
- How will we automate the contributor being added to the appropriate Jira and wiki group(s)?

A couple comments:
- What's with the blue bar at the top of the document?
- Gotham looks good as a heading/accent font, but we should probably use something more readable and readily available for body text
- What's the situation with the licensing of Gotham? Do we have a full web license? I talked with @jonong1972 about this a while back and we decided to use Montserrat on Google fonts which is nearly identical.
- If we're doing auth with GitHub, can we pre-populate much of this data from the Users API endpoint?
- Most people probably don't know what a JIRA is. That field should probably say (or at least have help that says) something like "user name on tickets.opscode.com."

It was a copy-pasta from earlier mockups I did of the community site. I was going to add a link to a printable PDF in that bar somewhere, but didn't get around to it.

Yup 😄. Although we may have to check with @btm and legal. Even if we can pull their full name from GH, we may still have to require they type it in for "legal reasons", ya know?

I think that field is dumb. I wasn't thinking when I set it. That's actually not something this site should keep track of anyway. I think it's more likely to become "Phone"
