Seems that known uses of kv got the sequence of Create and Open right. If anyone needs this, please reopen.

I retracted a comment that was based on the false intepretation that the kv package already had that functionality. I know of perfectly reasonable use-cases where it's desirable to be able to use a named file if it already exists, or create it otherwise, without needing special "does this database already exist" logic. Some examples would be a cache, or as I'm considering use of kv for, a set of ad-hoc indices for a flat-file database. Of course, CreateOrOpen is only a convenience, and doesn't really provide anything that we couldn't do in a few lines ourselves.
