Update:

I've fixed this.

I realised it was the "#" in the markdown code sections (the ones created with triple backticks)

You can't have any "#" (h1) in markdown sections, otherwise the table of contents will mess up.

Removing the "#" in the markdown code sections fixed my problem.

Old:

```
# Some comment
def some_code_here():
  ...
```

New:

```
Some comment <- removed "#"
def some_code_here():
  ...
```
