Option 2 has the following difficulty: if a series has per-file lookup table data, then the vtkDICOMApplyPalette filter will need to get the FileIndexArray from the reader, in order to map from slices to file instances in order to get the right lookup table for each slice from the meta data.
The FileIndexArray could be inserted into the FieldData of the vtkImageData. Even better, could it be inserted into the vtkInformation? Could the same be done with the meta data itself?

Okay, a rather extravagant fix was used. The FileIndexArray is now a member of the vtkDICOMMetaData, which in turn is carried along with the pipeline information. Yes, the DICOM meta data is now part of the VTK meta data. The patient matrix is also carried along the pipeline.
The new vtkDICOMApplyPalette filter uses the meta data to apply the correct lookup table. For now, it only applies a palette to images with PALETTE_COLOR images. It does nothing to images with a supplementary palette or enhanced palette.
