Have you used the correct env settings?
See our docker-compose we use for our registry (we can delete our manifests with these settings):

I have not had good luck directly deleting the file structures. I followed the docs to the letter for deleting via the API and then running the GC. This works great. We add about 2 to 10 images daily and we delete older images every 3-12 weeks. It has been working for more then 6 months now.

I'm removing the files via the filesystem, and then calling garbage collection to remove the blob's. But as far as I can tell the DELETE operations at least for manifests do not work.
