I can confirm that there is an issue in this code when you are using two or more seperate formsets on one page.

I have multiple tabs, all with one formset each, and different prefixes.
The first tab, works as expected, all other tabs, show only the add button.
Are you still working on this? It appears that you're not properly seperating multiple instances of your plugin, but I'm not versed enough to solve the issue.
