Use node-dev and point it at ./bin/www and it will work just fine.
node-dev ./bin/www
If the new project layout is too confusing I might put it back, but the idea is that it separates running from defining the server which let's you setup tests easier.

It seems that nodemon ./bin/www works as well so I don't see that the original issue is.
