In order to keep multer backwards compatible, the default is to write to disk. The inMemory option gives the choice to not write files to disk. When inMemory is true, no write to disk occurs and it is left up to the programmer to explicitly write files to disk, if they want to, via file.buffer property. Also please be aware that if you use npm packages, the multer npm package is a little older than the latest Github source -- still waiting for the inMemory option to make it to npm.

Is there any ETA for the availability of the latest version on npm?

The npm maintainer requested that I include more mocha tests than I originally had, which I did late Saturday night. Once he is satisfied, he'll update npm.
