The TOC is fixed. There wasn't a problem, it was just that when we made Moby-Dick into an epub3 file, we couldn't be bothered with adding the 135 chapters as a table of contents. I wrote a python script to do it and the the full table of contents for Moby Dick have been uploaded.

Sorry, but at least the first 2 issues are still open!
- The TOC is incomplete
- The layouting does not work, when left pane is shown.

Hmmmmm, I think what's happening is that your browser has the book stored in local storage and is not checking to see if there is a new version of the TOC. THAT is an issue, but I did add the complete TOC the other day. Try clearing all futurepress cookies, sessions, etc. and you should see the new TOC.
As for the layout when the left pane is shown, I will also raise that as an issue... I was thinking about how best to go about that.
