In 20eb3a2 the logic was changed to use the highest issue count seen while running the iteration.
The only instance I can think of where this would be ineffective would require a low initial count, only very recent samples being selected, and one of those samples having a large issue count increase.
I don't think further mitigation should be necessary, but a possibility is to use the changes between samples in the threshold as well (e.g. must be 10 times either the highest seen issue count or the greatest change between samples)
