It's not easy to calculate bytes_in, which is harder than bytes_out.
Bytes_in is the size of request header plus size of request body. As you mentioned, nginx core structures don't provide the receive data. Some change to nginx core function is essential.
In some situation such as file uploading, size of request header is usually smaller than request body, the 'Content Length' header can be used to calculate the approximate value.
