Re-opening because the layers should also be de-duped if possible and collapsed into a single layer.

That brings up a related question: would it be possible (even, easier?) to dump to an actual featurecollection rather than CSV-style rows of json geometries? In the featurecollection case this would likely be faster to read by Mapnik.

Prototype (just sample code in tests) added in #6 that uses Mapnik 3.x to avoid the need for verbose XML. If this approach looks good to you @tmcw then I think it obsoletes finishing this issue.
