I just merged some init script changes to master that address some of the errors.
The nagios.configtest file has been moved to Nagios' var/ (PREFIX/var or --localstatedir for configure), it didn't really belong in /var/run/ anyway. This may be removed altogether in the near future as it doesn't add much, and its implementation has been problematic.
The "/var/lock/subsys/nagios" file is the 'subsystem lock', used mainly by rc when changing runlevels. We need this so our init script will be run when shutting down, but it's not needed for your non-root case. Passing --with-lockfile to configure actually sets NagiosRunFile, the nagios.lock file holding the PID that the nagios executable itself uses to prevent more than one instance from running.
The init script is intended for the system to control the lifetime of a nagios daemon, and assumes it will be run with superuser privileges. For situations like yours, a separate script may be more appropriate, and simpler. It would also be handy for development.
