This isn't an API that is currently built into iCloudDocumentSync. However, to determine which files have changed you'll need to maintain and update your own data source. By maintaining your own data source you can determine which files change after a file update. This may possibly be implemented in a future version.

UPDATE: For the time being, this feature will not be implemented in iCloud Document Sync. The way that the iCloud queries are currently setup, this cannot be support. You may, however, find the NSMetadataQueryUpdateChangedItemsKey API (available in iOS 7.0+) useful if you're willing to plunge into the source code (the private enumerateCloudDocuments method is where this could be implemented). If you do implement it, please submit a pull request (would be much appreciated). For now, I am going to close this issue.
