The error is from GitHub's API. You cannot approve a PR with the same user account that the PR was created by. You can solve this by using a PAT associated with a different user, instead of using the default GITHUB_TOKEN.

If you are setting this up in a org, my advice would be to create a GitHub machine-user account that is only used for "bot" operations. Create a PAT on the new user and use that PAT as the token when you approve the PRs.
