I managed to get this working, the issue was related to a cache issue in Shopify App Bridge and is not related to this project in particular

How are you attempting to install the app? Please list the steps and what is the full URL used for the installation?

@waqas61 I don't remember unfortunately but try disabling or clearing the browser cache and try again
