I missed something obvious here, the iptables concurrency issue.
So iptables (the command) is not a realistic way to handle a very large amount of concurrent clients.
Instead I'm using ipset now. Ipset creates a set of IP-addresses in the kernel that you can add to or remove from concurrently.
Iptables then uses the set in matching rules. I'm modifying my manage scripts to reflect this change and closing this issue.
