Yeah, I've encountered this too, though not when I was in a position to stop and try to figure out what was happening. Are there specific steps that are guaranteed to reproduce it? Looks like it's something to do with the HMR manifest not being generated in time or something.
Without error handlers in preload it'll just serve a 500 if something goes wrong, it shouldn't cause a hang. I tend not to bother catching the error, I just take the 500 (though Sapper could probably do a better job of dealing with them client-side)

Whatever this bug was, it's most likely fixed in either 0.7 or the (soon-to-be-released 0.8), since all this stuff has been overhauled. So I'll close this.
