As often, the problem is in front of the screen .... ;-)
My Raspi cannot browse to www.amazon.de.
I assume once I solved this, the refresh will work.
Issue closed for now, sorry, to bother you.

I can't suppress the message Error: You can try to get the cookie manually by opening http://localhost:8080/ with your browser. / null It's part of the alexa-cookie app, telling you to actually open the browser for login (which you already did).
If you use the REFRESH_TOKEN env variable or the SET_REFRESH_TOKEN option in the script EMAIL/PASSWORD are ignored. The refresh token must contain the entire string starting with Atnr|....
Could you post a redacted version of your /tmp/.alexa.cookie.json.

The .alexa.cookie.json is zero byte length and empty.

Check the string length of your token. Mine looks like this:

If you're NOT using a German Amazon account you would have to supply additional options to the alexa-cookie-cli.
my only guess is that something went wrong while copy+pasting the refresh token.

I am using a German account with amazon.de
Length exactly same, pattern is different:
