can we use a shieldsymbolizer with text-dy or dx for this, or just a point symbolizer and textsymbolizer (and some will be lost)? Getting some points is better than drawing none to ensure the most possible consistency with the mapserver output.

I like the shield route, since mapserver is always drawing points. How important is the text-label-position-tolerance - should we get a shield-label-position-tolerance as well?

label-position-tolerance
I don't think is important. It came up in relation to text along lines (LINE_PLACEMENT
) in the sense that it was expensive and needed to be turned off by setting to zero. If the look is not right you could also try a point symbolizer plus text-symbolizer along with placement-type="simple"
