In turning inline disabling back on as an option, I'm finding some correctness bugs with outlined programs. I vaguely remember this coming up and it being part of the reason why I turned off outlining as an option. Time to dig through the commit logs.

Found a couple of quick fixes and everything is mostly working. Probably still a few more to hammer out before things work perfectly.

Man oh, man, did I find some bugs. Your run of the mill, small logic error bugs, but also some pervasive semantic error bugs that just happened to cancel each other out.
Outlining is back. It works with optimizations. And you can do it on a per-instantiation basis now.
