If instrument exists give options:

1. cancel
   1. stop the whole import
2. cancel just this instrument
   1. continue but ignore/disregard the instrument in question
3. replace completely
   1. erase the original version of the instrument completely, then add new one like normal
4. keep old fields but replace existing ones
   1. like the previous option except keep any fields that exist on original but not in new
5. replace but keep old fields
   2. Keep all overlapping fields from the original instrument, only add fields from new one if they do not exist on original
6. add all fields and give normal option for renaming duplicated ones (suffix)
   1.this is current default behavior if rename is chosen

php needs to tell js when it has converted a file
