Handle large directories

We handwrite the directory node today, and it doesn't handle the case where the directory node is too large (larger than block size).

Explicit `--outDir` and `--prefix` flags

  • Rename the --output flag to --prefix , have it simply prepend the carfile filename with the given string (it should not contain any slashes)
  • Add a new --outDir flag which will specify the output directory to put carfiles in, separate from the filename prefix

incorrect commp for files past the 1st after splitting
