I think the tool tips on the headers is the best idea. Once again, that minimizes the amount of "extra" text on the screen. Having the information available is important but we don't want superfluous stuff to distract from the data to be voted on.

From LN: It would be nice to be able to hover somewhere and see the full range of severity options. Especially since since worst severity since your OMT and past 2 days is different (mild, moderate, severe, very severe) than the week before (No Problem, Mild, Moderate, Severe, Very Severe). The only reason I know those is because I have the luxury of looking at the form for reference.

I think there's a typo in the Patient-reported Severity tool tips. Should be "No Response or blank ..."?
