Currently with a theme we are adding menu-3-col, menu-4-col etc to specify the boxmenu item width.

Audio can be removed as it has been added to the audio extension.

Repo name can be renamed so authoring tool changes may not need to be implemented.

Returning to a menu via a bookmark shows all buttons in their original state, regardless of whether a page is complete or visited.

This will become technical debt once deltanet/adapt_authoring#66 is fixed.

The page image is missing in some circumstances.

If there are no assessments in the course the image can dispear on completion of a page and return to menu.

To reproduce, create a course with a number of pages in the root of the builder, i.e. with no submenu. Choose the deltanet boxmenu.

Does not prevent user progress on it's own but combined with the home extension the page does not load.

FW v5 updates are on OS-v5 branch. Need menu item widths applying. Also uniform item height.
