Image on card looks awful

Filtering for questions on the sidebar

Subject, topic, skill

Maybe drop downs like

Select Subject
Select Topic
Select etc

Display number of where it is on card, so the user knows it's already added to the sheet and also what number(s) it is.

Add question button on card not centered.

Either select multiple questions and add random 10 from them OR in preview shuffle order.

When there are a lot of questions, there needs to be solution.

Pagination could work - but also there could be a button on the floating sidebar/topbar that says back to top or something

have each question display individual difficulties on it
when you hover over the difficulty, change the image on it
similar to a "shoe size" - this will reduce the amount of questions showing up
also add a nerdy tool tip explaining the "difficulties"
Still keep card with image on top, then difficulty options, then plus or minus, then submit
