I had a very similar problem. Assuming you've setup your Twitter application and placed the key/secret in the right location, the missing piece was to include a Callback URL under your Twitter application's Settings. That resolved the issue for me. Please let me know if that works for you.

Also I should have set Read/Write app permission as well.
