"Inclusive" is the source of infinite confusion and should be replaced.

the "First Name" label does imply only use the First of n and the Last Name perhaps the last of the string?

Hi Kirk,
I like it. Nothing comes up for "Place" or "Record type" (I think
it needs to be all or none) and "Search nearby places" needs to be
"vicinity".

Very useful. Do you want suggestions for other places where advice is needed? Also, do you want improvements in wording? For example Firstname should start with "Optional:"
On a similar topic, should we add a red star after fields which are mandatory. This is common practice in other websites.
