I noticed there wasn't an answer for question #3, I'd be happy to submit one.

Question 4. Elements owns index.

for (let i = 0; i < length; i++) {
    array[i] = typeof defaultValue === 'function' ?
      defaultValue(i) : defaultValue;
}

Suggestion:

for (let i = 0; i < length; i++) {
   if(typeof defaultValue === 'function'){
      array[i] = defaultValue(i)
   else{
      array[i] = defaultValue;
   }
}

In README.md, filenames, etc.
