According to the plugin's Stats tab, most users of this plugin have Moodle 3.3 and 3.1 branches !

When a user try to change the password by himself the transaction will not succeed because the "change password" page does not has the token requested field.

Please fix !!

I uploaded the zip file ( auth_a2fa_moodle37_2019072800 downloaded today) to my local moodle installation and after installation it immediately prompts me with "Upgrade key required" and I can no longer log in to my site. I've not had a chance to configure anything or follow any onscreen instructions as this was displayed as soon as the plugin installation was completed. Can you help please?

When trying to generate a new key, it only works for admin accounts. I reproduced it on two moodle installations. I also did a debugging of the javascript call to the php script. When logged in with an user account, nothing (empty response) is returned from '/auth/a2fa/generate.php' and the 'res' function is also not called.

Please assist.

I am having two problems with the TOTP factor. If I try to set it up via the link under preferences for my user, I get this error: Exception - Call to undefined method tool_mfa\local\form\setup_factor_form::xss_whitelist_static_form_elements()

If I try to set it up through profile instead, I get a QR code, but neither Authy nor MS Authenticator recognizes it as valid. Here is a screen shot of a QR code as generated:
