"File > Revert to Saved" is the standard OS X location for this feature, and it's already present :)
A checkbox somewhere for life updating would be cool though, and not too hard I think. There's an Spotlight API or something for getting notifications from the OS about that.

"Revert to Saved" indeed does work, and the warning dialog also makes sense in the use case that it is made for; reverting unsaved changes. A flag for eagerly getting changes back from the file as they happen, would be a very nice addition, I think.

Alright, there's now a flag "Edit > Reload Unmodified" that will live-reload a file provided that it has no unsaved changes.
I don't see any obvious way forward wrt attempting any live behavior on files with outstanding changes, especially since maintaining good performance of large files is important for HexFiend. Clearly more can be done here, but this should handle the original use case of watching changes to a binary file.
