Composition sections should be ordered (oe, ahdis)

The sections in the documents should have fix order equivalent to CDA-CH-EMED:

  1. Medication Treatment Plan Section Content Module (or other depending on document)

Das Element Organization.telecom ist mit der Extension nullFlavor dargestellt. Das war ursprünglich mal wegen dem Mapping FHIR-CDA so vorgeschlagen worden. In den Bundle-Beispielen hat man es aber angepasst und wenn es keinen Wert hat, wird das Element auch nicht abgebildet. Vorschlag, das Element aus dem Organization-Beispiel ebenso entfernen.

The CH EMED Observation Profile used in the Pharmaceutical Advice should be able to change the strucutred dosage for a referencing Medication Treament Plan. As a proposal we could create a "contained" MedicationStatement that is referenced inside the "partOf" Attribut of the CH EMED Observation Profile.

Medication.amount and Medication.ingredient.strenght are of Type Ratio where you have numerator and denominator as Quantity. These elements should also be constrained by http://fhir.ch/ig/ch-emed/StructureDefinition-ch-emed-quantity.html that the units are defined accordingly.

This would apply to all Medication Profiles.

Der Begriff "actuation" kann sowohl für einen Hub (Lösung) als auch für einen Sprühstoss (Aerosol) stehen. Die Übersetzungen "Betätigung (Sprühstoss)" sowie "bouffée" lassen jedoch nur die Verwendung als Einheit für ein Aerosol zu. Damit steht aber keine Einheit für z.B. eine orale Lösung (wie Tramal mit Dosierpumpe oder auch Flatulex) zur Verfügung. Wir bitten um Übersetzungen auf derselben "Ebene". Konkret sollte die Klammerbemerkung bei der Deutschübersetzung weggelassen werden, also lediglich "Betätigung" (allenfalls ginge auch Hub, weil Hub sowohl für eine Dosierpumpe als auch für ein Aerosol gebraucht wird). Im Französischen darf es ebenfalls nicht "bouffée" sein - da können wir aber keinen konkreten Vorschlag machen (dazu braucht es einen französischsprechenden Experten), Unklar ist für uns auch, ob die italienische Übersetzung sowohl für Aerosole als auch Lösungen gebraucht werden kann. Für allfällige weitere Erklärungen stehe ich gerne zur Verfügung.

For the provided examples the included "original representations" are just PDF's, and not in PDF 1/a format. The same PDF's are also used for the corresponding CDA-CH-EMED examples.

Currently the MedicationStatement.identifier is 0..*, however we need to have a unique uuid to identify this MedicationStatement in the exchange format (see CDA-CH-EMED: https://art-decor.org/art-decor/decor-templates--cdachemed-?section=templates&id=2.16.756.5.30.1.1.10.4.34&effectiveDate=2019-12-11T11:31:52&language=en-US). Propose to require one identifier with a uuid (system fixed).

This would also apply to MedicationRequest, MedicationDispense and Observation (for Pharmaceutical Advice).

In addition the document itself needs also uuid as an identifier, since this is related to all EPR exchange format this has been added as a CH-Core issue, see hl7ch/ch-core#95

In different codings are the system as "fixed Value" defined. This way multiple codings with different systems are not allowed. This should be changed, so that it is possible.

According to the CDA Spec (based on IPAG) the formCode for medication has a cardinality of 0..1.
FHIR is to strong with a minimum cardinality of 1

Change Medication.form to minimum cardinality of 0 in:

  • MedicationRequest

  • It shows a shortlist of units from our HIS that has emerged in Switzerland over 20 years.
  • With the value sets of units and galenic forms as used in the Hospindex.
  • With an analysis of how often these units and galenic forms are used in the Hospindex products.

We would like to put forward the following proposal for discussion: In order to do something against a free-text medication registration, which is rather undesirable in the structured medication transmission, the coding of medications could be required as follows:

If no GTIN or pharmaceutical code is available for an article, then at least the active ingredients should have to be indicated in coded form (by means of SCT substance codes).

Such a constraint could be modelled as follows:

Can you please check this out?

Structured Dosage can be of different types.

For normal dosing one Structured Dosage is required, for split dosing (any probably also tapered) multiple dosages with a sequence number can be provided.

Proposition to create two profiles, one for NormalStructuredDosage (max 1 and no sequence number set) and one for SplitStructure Dosage (multiple and sequenceNumber set).
