We have seen that the plugin creates the pagelist prefixing the file paths with ../.

The fact is that toc.xhtml file is on the same level as other files:
So entering ../ leads the program to search in the parent folder, where the files are not located.
I tried to test this on a Mac (10.15.7) using InDesign 17.3 and just got a javascript error:

We are using the plugin, we realized that when inserting the pagebreaks the attribute epub:type is inserted, but not the role attribute:
<span epub:type="pagebreak" id="page_2" title="2"></span>
It should be:
<span epub:type="pagebreak" role="doc-pagebreak" id="page_2" title="2"></span>
The role attribute is very important to make assistive technologies recognize the page number.
