Patients Lab Test Request

Patients Lab Test Request: Where patients can request for a test by choosing from a lists of available test... The patients sample will be collected from his/her house.
-patient should be notified of details on what he requested
-the admin will be notified too of what was requested
-Lab test will be sent to the patient's dashboard and printable as pdf.
-etc...
