Currently polyline strokes are not width-doubled. Blue sky:

• Upstream geojson-mapnikify, makizushi use composed svg markers (???)
• Less complexity with @2x raster marker caching, just use a single set of SVG and use the mapnik scale param

@yhahn what's the best way to initialize this kind of tilelive provider? a url, or a local file path, or raw data? All three?

Right now layers will not have any symbolizers if they don't have simplestyle

-simplestyle seems like a bad name for this. I'd rather do something like -simple.

@springmeyer does it make sense to use mapnik-pool with this module? would that give a perf boost?

They aren't getting added, somehow/somewhere.

@mapbox/geojson-mapnikify version should be updated up to 2.1.1 to use updated agentkeepalive version and prevent following error: node-modules/agentkeepalive#35

Should be pretty simple to implement

This will currently create one style per feature, even if features share styles. This is less efficient than it should be.
