Apparently having a custom client.onRequest(RegistrationRequest.type, ...) will override VS Code behavior (I guess you can only have one listener per request?). Not sure if this is a bug or if it's intended.
Removing the custom listener fixes my issue!

There can only be one registration. This is by design because having multiple listeners makes thing very complicated if they return values.
