Create wiki pages for each plugin, move the current info on readme to their own wiki page.

As well as an example of how to use it.

The form currently relies on the user already having a working captcha route but it will be handy to provide a generic route if there isn't one already defined.

Perhaps adding a generic form route too? but that seems way to overkilled.
