external link icon only for links spelled out in html

Right now if you put in an external link using
<a href="google.com">the link</a> you get a nice little icon showing that the link will open in an new tab.

However, would be nice if you could also get the icon if you specify the link using markdown:
[the link](google.com){target="_blank"}

update glossary documentation

It would be good to mention two things in the style guide about the glossary:

  1. that internal links (links within the glossary) are accomplished using the [term](t.html#term) syntax;
  2. that you must compile your book after you compile the glossary for the 'pop up' glossary definitions to work.

automatic extraction of functions / packages used on a given lesson page

I think it would be useful to purl() out the R code in a course page and determine which packages and functions are being used and automatically generate an index at the top of each page ("The lesson introduces the R functions t.test(), ezanova::Anova()" etc) as well as a global index for the course books so that students can find examples of where various functions are being used. Maybe there is already some kind of indexing functionality for bookdown we could take advantage of.

striped table example not striped

For whatever reason, the striped table in 2.1.4 is not coming out as striped. Maybe the kable and/or kableExtra packages have to be loaded rather than referenced with :: syntax?
