At line 333 there is a call to add_data_elevation. When invoked the function pukes at line 121
value = data[index['Report Result']] + " " + (get_qualifier(data[index['Validation Qualifier']],data[index['Lab Qualifier']]))
throws a Key Error for Validation Qualifier .....

Looks like this was intentional as it was commented out under the definition of COLS
