Missing header in decompression when static table entry is indexed and referenced header is evicted

When static table entry is indexed and as a consequence the referenced entry in the header table is evicted, it is missing in the decompressed header set.
Here is a script to reproduce the bug:

var hpack = require('../lib/hpack');

var ctx1 = hpack.createRequestContext({ huffman: false });
var ctx2 = hpack.createRequestContext({ huffman: false });

var buffer;

var val = '';
var i;
for(i = 0; i < 4096 - 32 - 1; ++i) {
val += '1';
}

var headers = [
  {
'x':    val
  },
  {
'x':    val,
':method':       'GET',
  }
];

buffer = ctx1.compress(headers[0]);
console.log('Compressed:',   buffer);
console.log('Decompressed:', ctx2.decompress(buffer));
console.log('----------');

buffer = ctx1.compress(headers[1]);
console.log('Compressed:',   buffer);
console.log('Decompressed:', ctx2.decompress(buffer));
console.log('----------');

The second header includes ('x', '11...11') name/value pair, but it is missing in the decompressed header set.
