Table of Authorities sometimes fails to generate

I am using the following citations:

and the margins:

When the table is generated, it produces the following: (Not all citations are used as of yet)

and if the margins are changed to the original, the following is produced:

I am not sure why a few cases are being indented and others are not. I am also not sure why barely anything is printing when the margins are changed.

File `.bluebook-doc.tex' not found.

The instructions were not clear about how to create the .bluebook-doc.tex file. It was pointed out by a third party, that the file must be created in the console.

%NOTE: This file is self-documented, and when all lines marked with %% are pulled % out into their own file, they form a valid latex document. To make the docs: % cat bluebook.sty | sed -n 's/^\%\%\(.*\)/\1/p' > bluebook-doc.tex; pdflatex bluebook-doc.tex

After running the above code, the file bluebook-doc.tex was created, but when I try to compile lawtex-doc.tex, I am still getting the LaTeX Error: .bluebook-doc.tex not found
