One common cause of release related issues is when the scm location of the project doesn't match the scm location in the pom, which means tagging the project will actually tag the incorrect source code when you would typically handle release:perform.

The release plugin should block this (maybe it does via config?) but appears to not do this by default. This plugin should block the release if these locations are different.
